Insurance
We are committed to helping our patients maximize their benefits and we will work with you to acheive the maximum benefits for your coverage. If you have dental insurance we will complete and submit a claim form as a courtesy to you. We also ask patients to be responsible for the estimated portion their insurance does not cover at the time of service. The patient is personally responsible for all fees for services provided, not the insurance company. We will wait 90 days for the insurance company to pay their portion. In the event the insurance company fails to pay, rejects the claim, or does not pay the total balance, we ask that the patient make arrangements for payment. We are happy to discuss any finance questions or problems and find possible options.
Insurance we accept:
Aetna, Ameritas, BC/BS Arizona (non-Federal), Cigna (select plans), Connection Dental (GEHA, Guardian, Lincoln Financial, United Healthcare, others), Delta Dental, Humana, Met Life, Principal, Sun Life Financial, TDA, United Concordia (select plans), Veterans Administration. We accept all indemnity plans and most PPO plans. Please check with us for verification. AHCCCS, HMO and Medicare plans do not pay our office.
